Tiffany J. Riser currently serves as a Registered Nurse Practitioner (RNP) at the VA Palo Alto Health Care System in the Home Based Primary Care (HBPC) Program. She provides primary care, palliative care and case management for homebound, hospice, assisted living facility and residential care facility patients that have been identified as fragile or vulnerable and medically complex with barriers to accessing the traditional outpatient clinic. She enjoys the home care setting because, as a provider, it allows her to obtain a more clear picture of the environment in which older adult patients are being cared for, observe the challenges faced in their day-to-day lives and develop more personalized goals for their care that take into consideration their unique preferences and circumstances. She is board certified as an Adult Nurse Practitioner by the American Academy of Nurse Practitioners (AANP). She earned her Bachelor of Science in Nursing degree from the University of Pittsburgh in Pennsylvania. Ms. Riser earned her Master of Science in Nursing degree and Master of Public Health degree from the Johns Hopkins University in Baltimore, Maryland. Her Adult Nurse Practitioner training program included clinical rotations in primary care and internal medicine settings and her MPH concentration focused on community health. She presented the findings of her research projects focused on assessing domains of primary care provider accessibility among state Medicaid enrollees and the relationship between continuity of care and diabetes care measures. Ms. Riser’s current health policy interests include integration of and access to palliative care services, healthcare delivery and access for aging veterans and innovations in dementia care.
