Dr. Tracy Fasolino is a Professor and Distinguished Palliative Care Leader for the School of Nursing at Clemson University in Clemson, South Carolina. She is a 2021 Cambia Sojourn Scholar with a vision to address the health needs of rural residents in Appalachia. She has an active clinical practice as an embedded palliative care APRN in a pulmonary practice, having spearheaded this community-based, first-of-its-kind, nurse-led model of care to address the needs of those with advanced lung disease. Having completed a graduate certificate program in policy studies, she serves as the S.C. State Ambassador, Chair of the Public Policy Special Interest Group, and the Board of Directors for the Hospice and Palliative Care Nursing Association. She also serves on the steering committee for the S.C. Coalition for the Care of the Seriously Ill and actively engages community leaders in education and training on public policy related to advanced care planning and end-of-life.

Dr. Fasolino’s research focuses on improving care delivery for dually enrolled individuals (Medicare-Medicaid) living in rural areas diagnosed with advanced lung disease and their caregivers. Her studies are designed to engage rural clinics and community services to address the growing needs of primary and specialty palliative care using an embedded model. Dr. Fasolino’s health care policy focus is addressing the challenges and barriers these individuals (nearly 2.5 million U.S. citizens) face when attempting to access services for symptom management and coordinated care.
