Elizabeth Fine Smilovich, MD

Elizabeth Fine Smilovich, MD is a practicing Geriatrician at the Cleveland Veteran Affairs Medical Center and an Assistant Professor of Medicine at Case Western Reserve University School of Medicine. She is the medical director for the Cleveland VA Geriatric Outpatient Clinic and is a member of the VA Outpatient Geriatric Programs National Workgroup. She has developed curriculum for medical students and interprofessional learners focusing on an age-friendly approach, interdisciplinary collaboration, and improving transitional care for older adults. She was the site-investigator for the Cleveland VA arm of the Transitional Care Model Replication Study.
